The Healthy Opportunities Pilot (HOP) is a program in North Carolina that provides select evidence-based, non-medical interventions related to housing, food, transportation, and interpersonal safety to high-needs Medicaid enrollees. It focuses on connecting people to resources beyond health care.

Social & Structural Determinants of Health (SSDOH) are the non-medical factors that influence health outcomes. They are the conditions in which people are born, grow, live, work, and age, and they play a significant role in shaping overall well-being. These factors can impact access to healthcare, quality of life, and the risk of developing chronic illnesses.

 

Key Categories of SSDOH:

  1. Economic Stability – Income level, employment, job security, and financial stability.

  2. Education Access & Quality – Literacy, early childhood education, higher education, and job training opportunities.
  3. Healthcare Access & Quality – Availability of healthcare services, health insurance coverage, and health literacy.
  4. Neighborhood & Built Environment – Safe housing, transportation, access to nutritious food, clean air and water, and crime levels.
  5. Social & Community Context – Discrimination, social support, community engagement, and exposure to violence.

KYHOM & HOP

KYHOM has partnered with DHHS to provide services to assist in addressing these determinants. We offer Holistic High Intensity Enhanced Case Management: Provision of one-to-one case management and/or educational services to address co-occurring needs related to housing insecurity and interpersonal violence/toxic stress, and as needed transportation and food insecurities. Case management related to transportation needs are included.
